Health and Personal Information Forms
These are required to be completed before your first class. The information requested is necessary to meet insurance company criteria, to ensure your health and safety during classes and to facilitate lesson planning around your individual needs. The forms are kept in a secure location and only taken out on the day of your course. Files are always kept safe under the supervision of your teacher. These documents will NEVER be shown to a third party or shared without your specific instructions. They are required for insurance purposes to be securely stored for seven years after your last class.
If you wish to alter any of your data, review how it is used or wish to request its return to you, please contact mary Griffiths on [email protected] You may, of course, decline to submit personally identifiable information to Body and Soul Yoga, in which case we may not be able to provide certain services to you.
How we use your contact information
When completing the Health and Personal Information form, you will be asked to sign a disclaimer. In doing so you are agreeing to your email and phone number being used by Body and Soul Yoga to inform you of class changes and service updates directly relating to the service you have booked.
You are also given the option of being added to the Body and Soul Yoga Mailing List and receiving emails periodically informing you of related services and information we believe you may be interested in. You can unsubscribe at any time by clicking on the Unsubscribe button at the bottom of the email. These emails are sent via Mailchimp, which is a secure server. Their policy can be reviewed at:
Emergency contact information is collected solely for use in an emergency, should Body and and Soul Yoga need to contact your next of kin or give this information to the emergency services. This is the only information that would be shared with a third party and only in the context of an emergency.
Notifying you of changes to this notice
If there is a change to Body and Soul Yoga privacy notices or how we use your data we shall contact you via email notices. We reserve the right to determine the form and means of providing notification to you, and you may have opted out of certain means of notification as described in this Privacy Notice.
